The pattern probably exists because in this universe, Aleister Crowley was a bona fide successful magician, and now he secretly runs Academy City. In the real world, when Crowley founded the Abbey of Thelema (which he intended to be a worldwide school of magic), his magical name then and until his death was "The Beast 666." So it makes sense that other magicians would use that name scheme afterwards.

It's an interface to access arcane power, or it's the name of your "true" self, or whatever your philosophy says it is. The point being, you're not John Smith when you're using magic. You're Lysander, practitioner of the craft, or what-have-you.
As for why they have to say it to their opponents specifically in Index... No idea. I didn't watch the show very far. :) But I hope I'm helping!
